Support Coordination
NDIS support coordination is a funded service that helps participants turn their NDIS plan into action. It focuses on making your plan easy to understand, connecting you with the right providers, and ensuring all your supports work seamlessly together. Support coordination falls under the Capacity Building section of your plan and is typically funded when the NDIA determines that you need additional help managing your supports due to your disability, living situation, or the complexity of your needs.

A support coordinator is an independent professional who works alongside you to build your skills and capacity. Their role is to help you understand your funding, feel confident in your choices, and become more independent over time. You remain in control of your goals, your providers, and how and when you receive support.
There are three levels of support coordination, and we customize our approach based on the level included in your plan:

-
Support Connection – Assisting you in building skills and connecting with providers.
-
Support Coordination – Helping you use your NDIS funds effectively and manage your supports.
-
Specialist Support Coordination – Providing extra support for individuals with complex needs that require more in-depth planning, risk management, and coordination.
